If they wear a dress, yes. For trousers, especially if the material is dark, they get to wear stockings to prevent staining.
My doll still has her default lingerie set on. It's white, so it won't stain her. I don't want her to turn black/blue/whatever color of her clothes, so that's why I keep it on. D:
I was always a "they don't need it" person. Last week, however, I took my girl to visit the ladies in an assisted living home. She had on a long historical type gown. Pretty soon the 85 year old ladies were lifting up her skirt to see how her joints work. They were a bit shocked by her anatomically correct parts. Now I am sewing her some underwear for when she goes visiting.
I only put underwear on dolls if they need it-- If they are wearing a skirt or something where they might be showing something they shouldn't! Or if there's a risk of staining. Or if there's enough room for it... (some pants are too tight for underwear). Otherwise, it's not as if they are human and need the underwear to feel comfortable or to keep their clothing clean! Underwear is mostly for being part of a complete outfit or for protection from staining, or for being "proper." OR for dressing up in lingerie or something sexy!
My mini boy has a pair of boxers, a pair of briefs, a thong, and a pair of bloomers. On the other hand, my 70cm boy has no underwear. I put underwear on my dolls for no reason other than I like underwear. As a plus, if my best friend tries to peek beneath my boy's skirts (he crossdresses) she doesn't go, "Eek, a penis!" -___-
Three of my boys wear underwear. I have seven. I'm not quite sure why some do and some don't.... but I'll probably end up writing that in as a character point in the future. I'm sure plenty of people don't wear it in their real lives, right?
Most of my dolls wear underwear or will (my Shoyo's underwear is in the mail, on its way to me, lol). Hakkai is the only one of mine who doesn't >>; mostly because he didn't come wearing any, and I like his pants so much I haven't taken them off, so I wouldn't have a chance to put any on anyway, lol. He doesn't seem to mind. xD Kazuki wears underwear and a bra, though, especially since she wears a lot of skirts. Fai wears leggings as pants all the time, so of course he has underwear on under there. xD
Neither of my MSDs (both boy and girl) wear any underwear - but that's simply because I haven't gotten around to buying them or making them specific underwear. The girl not wearing underwear doesn't particularly affect me, but I am more motivated to get the boy pants, simply because it makes me feel more comfortable. Both their outfits are in light colours, so I'm not too worried about staining. My girl wears shorts anyway, which are practically underwear, so I'm not too bothered about her. For me, underwear isn't a neccessity, it's just another item of clothing to buy your doll - unless you really think that they need it.
My boys wear underwear. Mainly because they have low rise jeans and you could see their butts when they sat down. If I had a girl, she'd probably wear panties, especially if in a skirt, but I'm not sure about a bra. It seems, since she'd be made of resin, she wouldn't need one, and it'd probably be a little bulky.
I put undies on my doll, but he came with them. I was planning on purchasing some even if he hadn't arrived with them on. It's part of his character that he is shy, so that is one reason. Another is that he has something to cover up with when I have him stripped down for a clothes fitting (my mom and I sew them). It just makes me feel better all around.
I don't have underwear for my Megi but when I get a girl an she will wear a skirt or a dress, I want to buy some lingerie for her.
Some of my dolls wear underwear, some don't. Sometimes the clothes are too tight for underwear and then they dolls don't wear, sometimes I just don't have the suitable underwear for a certain outfit.
My girl, I put underwear on her. That's also cuz she mostly wears dresses. But if I had the briefs I'd probably put my male in it, just for fun. Do I feel weird if I don't put them in it? Yeah, a little but that's cuz underwear is so much fun >< But yeah, definitely on dolls that wear skirts or dresses. ^_^
Yes, I do. My girl doll I don't really because it's hard to get pants on, if she's wearing a skirt or dress I usually put panties on. My boy dolls all wear underwear. Mostly so I don't pull down their pants and see BAM!PENORZ and their junk doesn't stain.

I have all boy dolls and they all wear white underwear and white socks! I just dont want them staining, or to catch a cold, LOL!
My doll came with underwear so she is wearing that but if I bought a doll that didn't have underwear I would buy some because I like the realism in them wearing under wear.

Right now, no I don't. At the moment the type of dolls I have just wouldn't be able to fit underwear under their clothes. I could use some panties for skirts though. When I someday get a mnf, she will probably wear underwear with whatever clothes will fit. Also, boxers or panties with a tank top can be cute pajamas! Edit: also, some girls seem to need bras. Depending on the detail in the chest area, sometimes their shirts get a bit see-through! O.o
